Before you can be considered as a chaperone for a field trip, you must be approved in advance.  This also applies to the alternative person you select.  No individual will be allowed to chaperone (supervise children other than their own) unless they have submitted a completed application and been approved as an appropriate chaperone.

Click on this link for the chaperone/volunteer form.  You can fill it out and return to the main office, or your child's teacher. 

*Important!  Please fill out this form in the beginning of the school year. The form needs to be reviewed, references need to be checked and you must be approved by the Board of Education at their monthly meetings. This can take time.  You can not attend field trips without prior approval.
